Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

Insurance and Payment Information

You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office (805) 497-6979. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

Insurance Coverage

Advanced Foot & Ankle Medical Center has the privilege of accepting a wide variety of insurance plans for the plethora of procedures we perform. While we must remind you that every insurance plan is different, and coverage is not guaranteed, here is a quick rundown of the insurances we deal with. Most of the routine procedures we perform are covered by the insurance plans we accept, such as nail care, matricectomy, nail debridement, x-rays, orthotic casting and dispensing, wart treatment, low dye taping, physical therapy, kinesio taping, and more. Also, we perform most foot and ankle surgical procedures, such as hammertoe correction, bunionectomy, cheilectomy, and foot and ankle fusion. Again, please contact your insurance provider for the specific details of what they do and do not cover from our office.

Here is a list of some of the insurance plans we accept, if you do not see yours listed, please call us to see if we accept your plan. These plans we accept can change over time, so while this list will always remain accurate, it is not guaranteed.

Covered California Insurance- we do not accept insurance plans that are under Covered California.  Typically, the insurance card will say “Pathway” at the bottom right corner of the card.  If you have any questions, please call our office.
